Form Builder Field Types Explained

Every field you can drag onto a form in Form Builder, and what it is for. Fields are grouped here the way you will reach for them: collecting answers, structuring the form, capturing proof, and logging operations.

Answer Fields

• Text for short answers, Paragraph for longer notes and observations.

• Number, Email, Phone, and Password accept only their own kind of input. URL places a clickable link on the form.

• Date and Date/Time let the user pick a date or date and time. Both have an auto setting that fills the current value and greys the field out.

• Address captures an address by dropping a pin on a map or searching. Location uses site detection to record which site the user is on.

Choice Fields

• Radio for two or three mutually exclusive choices, such as yes/no. A segment setting shows it as a segment bar.

• Checkboxes when the user may pick several of a few choices.

• Select (a dropdown) instead of Radio when there are more than three or four choices; Select Multiple instead of Checkboxes when there are more than about five.

• Score records a score. Pass/Fail is for fleet inspections: choosing Fail automatically creates an issue for the selected vehicle.

• Radio, Checkboxes, Select, and Pass/Fail can all drive conditional questions: follow-up fields that appear only for certain answers.

Structure and Guidance

• Header titles a section; Label shows static instructions. Neither takes input.

• Panel groups related fields. A panel can be required (the user must complete at least one field inside) and repeatable: for a room-by-room inspection, the user duplicates the panel once per room.

• Site Info displays the site’s notes, operating hours, and maps to the person filling the form. It only appears when that information exists on the site’s detail page.

Proof From the Field

• Photo launches the Sitefotos camera and attaches the photos to that exact section of the form.

• Signature collects a signature; Sketch collects a hand-drawn sketch.

• Weather records the weather at the moment the form is submitted.

Operations Fields

• Service logs a service performed on site. Configure your services first (under Services), then pick which ones the field offers. Service fields are what feed Service History and pricing.

• Material and Add Materials log materials used. Configure materials first (under Materials). Add Materials lets one field cover several materials, and the user can add entries on the fly.

• New Issue records a site issue with a photo and notes. Use it instead of a text field for issues.

• Crew lets a crew lead select crew members; the related crew panel records who performed which service and when.

• Vehicle, Equipment, Meter Entry, and Assign to support fleet inspections: pick the vehicle or equipment, record meter readings, and assign it to an employee.

Integration Fields

ServiceChannel, FMPilot, Corrigopro, Aspire, and API fields tie a form to an external work order system, for example to send check-ins or completion data back to it. They only apply when your account has that integration; Sitefotos support sets these up with you.
